Individual Support Individual support programs offer young parents: Information on child development, parenting education, individual counseling, advocacy, social support, referrals to community agencies and health centers, assistance with housing needs, education and relationship issues, Health for Two. P.A.T.H.S. (Partners, Advocacy, Teaching, Hope, Success)The P.A.T.H.S. program offers a range of individual and family support services to young pregnant and/or parenting moms who enter the program before they are 20 years old. Services are delivered primarily through home visitation. After the initial intake and assessment, an outreach worker is assigned for support. Healthy FamiliesHealthy Families promotes positive parenting and child development by providing long-term (up to five years) support to pregnant and parenting youth. Healthy Families supports healthy growth and development of children (physically, socially, emotionally, and cognitively), by encouraging parents to set and reach personal goals and parent effectively. Services are only offered to parents who are pregnant with their first child or who have recently given birth to their first child (child must be less than three months of age).
