PATHS Outreach Worker
Terra is a dynamic community-based organization that responds to the issues and challenges that young people face related to pregnancy and parenting. The P.A.T.H.S. program supports young moms in making healthy choices for themselves, their children, and their families and helps young moms to develop the capacity to effectively meet the needs of their children.
Responsibilities:
- Carry a client caseload of young moms and their children, providing service through in-home and community visits
- Assess and provide guidance/support/advocacy to assist teen moms to stabilize the home environment
- Provide pre-natal and parenting education and support as well as information regarding infant care and child development
- Link young parents to community resources for referral and/or advocacy
- Maintain files and complete required reporting, and record client information on the agency database
Qualifications
- Degree/diploma in the Human Services field.
- Minimum two years experience working with young people.
- Understanding of the needs and issues related to teen pregnancy and parenting.
- Experience in assessing client needs and identifying appropriate community resources.
- Knowledge of child health and development, positive parenting strategies and developmental milestones preferred.
- Comfortable using computers and databases.
- A reliable vehicle to transport clients and children is required.
- A criminal record check and child welfare intervention check are required.
